A child deed poll requires the consent of all persons holding parental responsibility.

Each organisation you notify will keep their copy. You'll need one for each place you're updating your child's name. Most people need 6–8 copies.

Changing your child's passport name
HM Passport Office may require a statutory declaration witnessed by a solicitor for child passport name changes, in addition to this deed poll. Your solicitor can advise — this is separate from and in addition to the deed poll itself.
