Change your name by deed poll
A deed poll is the legal document that changes your name. Fill in the three short sections below and we print it, post it to you and email you what to do next.
1. Your current name
Start with the name you have now. Your deed poll is accepted by HM Passport Office, DVLA, HMRC and all major UK banks.

Your deed poll is made out in your new name, ready for you to sign in front of two witnesses. You then use it to update your name with organisations such as the Passport Office and DVLA.

3. Where should we send it?
