As we explained in our article called Changing a child’s legal name, everyone with parental responsibility for a child must agree to the child’s name being changed before it can take effect.
In order for us to confirm that everyone with parental responsibility does indeed agree to the change, you must send us a letter, which is signed by all parents (we call this a letter of consent) confirming that all relevant parties are aware of and agree to the name change. We cannot release your child’s Deed Poll until this letter has been received.
Note: In the examples below we use the terms “mother” and “father” as an example role but these can be replaced by “legal guardian” if you are not the child’s mother or father.
For a mother with sole parental responsibility
This letter is to be used by a mother with sole parental responsibility for her child. Please note, on the template you will have a number of options to choose from for the second paragraph of the letter. You must read all options and only choose the one that describes your situation.

Signed by a mother and father with joint parental responsibility
This letter is to be used if both the mother and father have parental responsibility for the child. Please note, if the mother or father lives at a different address to the one you put at the top of the letter, their address should be printed under their signature and name at the bottom of the letter.

In order for you to send your letter of consent to us by email, you must have a scanner. Once you have scanned the letter, please attach the image to an email and send it to consent@deed-poll.org.uk. You should include your full name and post code so that we can identify your application.
