Letters Of Consent

When you apply to change the name of your child by Deed Poll, you must send us a letter of consent confirming that everyone with parental responsibility consents to the child's name being changed. The wording of your letter of consent will be different depending on who has parental responsibility for your child and where in the United Kingdom your child resides.

Please note, in the examples below we use the terms "mother" and "father" as an example role but these can be replaced by "legal guardian" if you are not the child's mother or father.

Signed by a mother with sole parental responsibility
This letter is to be used by a mother with sole parental responsibility for her child. Please note, on the template you will have five options to choose from for the second paragraph of the letter. You must read all options and only choose the one that describes your situation.

Signed by a parent with sole parental responsibility (spouse deceased)
This letter is to be used if you have sole parental responsibility for your child because your spouse is deceased and did not appoint anyone as legal guardian in their will.

Signed by a mother and father with joint parental responsibility
This letter is to be used if both the mother and father have parental responsibility for the child. Please note, if the mother or father lives at a different address to the one you put at the top of the letter, their address should be printed under their signature and name at the bottom of the letter.
