The below form is for a child Deed Poll. Once you have finished providing the required information, click on the submit button at the bottom of the page to be taken to the next step. After you have completed your child’s Deed Poll application, you must send a letter of consent to us signed by everyone with parental responsibility for the child. Upon receipt of your child’s Deed Poll application, payment in full and your letter of consent, we will process your application in accordance with the service you select at stage two of the application.
